Event Information & Registration
Directions for registering your MUD Team (please be patient as downloading these forms may take a few minutes):
- Team Captains must complete and sign the Registration Form (936 KB PDF) located in the event brochure.
*** IMPORTANT*** If you do not have all the forms completed, go ahead and submit your registration form with your payment now, in order to lock in your place in the tournament. Last year we sold out and expect to do the same this year. - Complete a Team Roster – must be completed with players addresses and contact information – 8 to 10 players.
- Each player must sign a Release of Liability Form. Make sure the signature is witnessed.

- To lock in your registration fee we must receive your registration form and check by the deadline dates posted. Roster and Release of Liability must be submitted no later then July 1 to allow time to receive your team wristbands for entry into the event.
- Please make sure everyone on your team is acquainted with the Rules of Play and Guidelines. They must be adhered to at all times.
- Be sure to include T-Shirt sizes with your team Roster. Sizes only guaranteed if sizes are received before June 24th.
- * Special for 2008! Collect additional donations for the Epilepsy Foundation and you’ll have a chance at winning a $500 Gas Card. For each $100 in donations collected, you’ll get one entry into the Gas Card Raffle.
